Incredibly minor error/nitpick while reading the Molydeus entry: It's init is +6, but it has 22 dex and improved initiative. It should be +10.

Small Oversight:

Your own lichfield template expressly mentions about hit dice:

Yes.....? I don't see the problem, Soul Harvester. The lichfiend DOES gain bonus hit points from Charisma. Do the math.

...Ah, my apologies; I failed to note that there was nothing expressly mentioned about the fiend losing it's Con score. However, on another note, I believe that either that you forgot to mention that a lichfield also uses his charisma score for fortitude saves; or that it's currently 8 points too low.

Do Stygian Linnorms actually live in the River Styx? If so, shouldn't they have the Extraplanar subtype?

I never give the extraplanar subtype to monsters, since it changes upon whether or not you are on your own plane. If you encounter a devil in Hell, it is you, not they, who have the extraplanar subtype. The Stygian linnorm in particular, isn't likely to be encountered anywhere other than the River Styx, thus it wouldn't possess the extraplanar subtype during such an encounter.

How does one determine how many tentacles the mind flayer has attached?

By how many tentacle attacks hit and succeeded in their grab check.

Basically it goes like this:

Round 1: Mind flayer full attacks. Grabs with 4 tentacles.
Round 2: Mind flayer extracts brain.

or

Round 1: Mind flayer attacks. Grabs with tentacle. Gains +2 bonus to CMB.
Round 2: Mind flayer pins. 4 tentacles are attached.
Round 3: Mind flayer extracts brain.

The second version also applies to any full attack that didn't have complete four tentacle success.

Any chance you're going to tackle Ulitharids (or whatever the big six tentacled illithids are called) or Elder Brains?

A chance, sure. But what are U-thids really beyond advanced mind flayers?

Well they have more tentacles, and more potent psychic abilities, and they are also extremely rare and considered a good omen for any mind flayer settlement. They are also on the low side of Large.

Sounds like you apply the advanced and giant templates and call it a day.

It's nice to see the elemental monoliths back. I always liked how there was an intermediate between the elders and primals.

The not-an-arrow-demon seems to be missing the effects of Improved Critical and of the +1ness of his bows in his attack line.

Also, that's a pretty large boatload of hit points for CR 7, no? Why so many?

I didn't change the ability scores from the original MM3 version, so it ended up with quite a few.

Naah. They aren't that bad. The PF succubus has comparable ability scores (except her focus is in charisma rather than constitution). And Kain seems to have boosted ability scores in the majority of creatures.

Alright, most of them are actually fine on second look, except the Con score is way too high for a CR 7, especially compared to other demons. Babaus, CR 6 demons, have a con of 20. Nabasus, CR 8 demons, have a Con of 22. Shadow demons, another CR 7, only have a 17. Even if it is supposed to be on the tough side, I wouldn't give it anything higher than a 22, which would drop its hit points to 115, still high for a CR 7 but far more reasonable. Alternatively, boosting it's CR to 9 would also do the trick. Maybe 8 if you're just that attached to it.

Yes, I'd say the Con score is too high as well. An uber-high Con doesn't even seem to make much sense for a fiend that is all about being a ranged-attacker. Let's save the crazy-high Con score for the higher CR, 'tank demon'.
