Our basement has three floor levels. Because of this, one of the cat doors is about a foot off the floor. Most cats can handle the jump just fine until they get old. We could leave the door open, but that allowed the litter box odor to spread. I though about building a step in front of the door, but it would have to be moved and repositioned every time somebody went through the door. Then I remembered a chicken ladder I'd seen. A few pieces of leftover oak, some salvaged hinges and a carpet scrap were all it took. A barrel catch holds it flush against the door when not in use.
