How to Obtain Military Discharge Paperwork:

• This is required to enroll in VA health benefits- no other documents are acceptable substitutions.

If your patient does not have his paperwork it is a long process to get it from the government. If you have to, though, here are the steps.

1) Go to www.nara.gov (this is the National Archives website)

2) Click search

3) Enter "standard form 180"

This will get you to the information you need. Form can be downloaded and mailed or faxed. Also, the veteran can apply online and fax a form. Requests can take 2-4 weeks to be processed.
