I am a single woman requesting K-1 Visa approval for my fiancé, Mr. ________, who is currently a resident and citizen of Canada. We met on the internet on DATE and had our first face to face meeting on ______. We decided to marry on ______________. We would like to reside in the United States. And are planning to be married ___________________.

Sergio is a single Canadian citizen from ________(city) who works as _______________. He has no children and has never been married before.

I am a citizen of the United States. I have never been married before and have no children and am currentely employed by _______________. I am financially independent, and manage my debts successfully. At the appropriate time, I will submit an affidavit of support which documents my salary and financial information for your review, as required by the law.

(I included a brief paragraph about why wanted to live in the U.S., but I'm not sure that's necessary)

Proof of our relationship, in the form of:
A. Signed statements describing our relationship;
B. Plane tickets receipts and boarding passes;
C. Train ticket receipts;
D. Photos of us together;
E. Copies of e-mails;
F. Copies of long distance telephone bills;
G. Holiday Inn Express bill;
H. License Plate numbers - we usually travel via automobile to visit each other and cross the border in Detroit at the Ambassador Bridge, so I have enclosed our license plate numbers for your information.
Proof of our intention to marry within 90 days of Sergio entering the US on the K-1 Visa, in the form of:
A. Signed statements of our intent to marry
B. A copy of the contract for our reception
