Clinical history

  • A 45 year-old man is seen in walk-in clinic for sudden onset of "anxious feeling" associated with shortness of breath and chest pressure.
  • PMH:
    1. PVC's and NSVT on Holter monitor in the past
    2. Heavy alcohol use
    3. Tobacco use, HTN, hyperlipidemia
  • Meds: ASA, HCTZ, ACE-I, statin
  • Exam: BP 153/88, HR 95, all else normal
  • Labs: ECG with NST changes, BMP normal, CBC normal except Hgb 17.3, Troponin-I normal
