You will need to get the 1-800 from your local Veteran Assistant, they will help you. If you haven't gotten a reply in a reasonably time check on it. I applied for one got it a year later . Due to lost the paper work and I sent it again, next it came in with the wrong side of the Civil War. That one I had to break into little pieces, next the freigh line broke it after carrying it on the freightline for a week. V.A. replaced it once again . The third time I pick up the stone at the freightline and we installed it. This Veteran Stone is not to be use with any other stone on the grave. They will tell you this. It is worth the effect get the stone. You will need form number QMB no.2900-0222. Hope this helps someone.
