Assessment Sample for a Fictitious University

Digital State University School of Arts and Sciences is improving curricula through the innovative use of technology inside and outside the classroom. iTunes and iPods were introduced this semester in our survey classes We plan to study how the use of these new technologies impacts teaching and learning in the School and how it helps to prepare you for your professional careers. 

Data collected from the following survey is part of that research and will be aggregated with that from other participating institutions to help inform a larger study examining how technology can best be leveraged to advance teaching and learning.

Please note that not all the questions may pertain to you, so answer only those that are relevant to your experience.

You must be 18 years of age or older to participate in the survey. There is no known risk involved in your participation. Your identity will be protected at all times. We will not share your responses or comments with your instructors and your grades will not be affected by your decision on whether or not to participate in the survey. We will assign a code number to your data and will use it only to track responses and compare changes over time. We will not use your name or any other identifying data in any of our data analysis procedures or reports.

(1) your personal comfort level with a particular device or software application;

(3) the degree to which you value that particular tool as part of your overall educational experience.

For example, you may personally feel very comfortable using an iPod, but your professor may not integrate it into his/her class, even though you feel it could be an extremely valuable learning tool if lectures were podcasted.
